My Application fee in the amount of $ accompanies this application.

FOR VALUE RECEIVED, the undersigned jointly promised to pay to the order of the sum of Dollars,
together with interest. Said sum, inclusive of interest, shall be paid in yearly installments of $ each, with a first payment due , 20 , and a like amount on the same day of each year thereafter until the full amount of this note and

accrued interest shall be fully paid. The undersigned reserves the right to prepay this note in whole or in part without penalty.

Full Regular Golf Memberships may be transferred only through the Club and pursuant to the By-laws, Rules, and Regulations. A membership

must be kept active (monthly dues paid), in good standing, and the application fee paid in full before a membership may be placed on the Club's transfer list If, and when, the membership is resold, an amount equal to % of the application fee in effect at the time the

membership was originally purchased (actual amount paid) will be paid to the transferring member.

I understand that, as a matter of contract with Owner, my membership confers upon me the right to use those Club facilities included within my membership classification, subject to rules and regulations established from time to time by Owner. While my opinions and suggestions regarding the management and operation of the Club may be solicited on occasion, I expressly acknowledge that my membership does not confer upon me any equity or ownership interest in the Club or its facilities or real or personal property any right to vote otherwise control decisions affecting the management, operation or ownership of the Club or of Club property or assets. I agree to be liable for the payment of the application fee, the applicable membership dues and charges incurred, by me, my family, and guests in the use of the Club.

I acknowledge that I have received and read a copy of the By-laws, Rules and Regulations and I understand that my membership rights and obligations shall be governed thereby, as said By-laws, Rules and Regulations presently exist and as they may be amended from time to time by Owner. I understand this application, once accepted by Owner, shall constitute a binding contract In entering into such contract I am relying solely upon the information set forth in this application, the By-laws, Rules and Regulations and the accompanying description(s) of membership classifications distributed by Owner. I am not relying upon any other representations, oral or written, by Owner or any agent of Owner, or upon any information distributed or communicated by any third party.
